Please be aware of the following:
If you have not received a Pre-employment Physical authorization form from Woodbourne, you may arrange for Human Resources to call the medical center and provide a verbal authorization for the Pre-employment Physical. In order to arrange this, you must contact Human Resources (443-901-0350, ext. 4206) during office hours and provide the name of the medical center you plan to use. The cost of the Pre-employment Physical is paid by Woodbourne Center, Inc.

• Employment is contingent upon the successful completion of the pre-employment physical, PPD, and Drug Screen.
• You must return to the same medical center from 48 to 72 hours of your physical to have your PPD test read.
