PROGRAM DESCRIPTION:
Short-term residential program for children who are at risk of psychiatric hospitalization, transitioning from hospitals, or in need of respite from residential programs, foster care or home settings

PROGRAM MODEL/SERVICES:

  • Respite care
  • Emergent and planned residential placement
  • Community-based treatment foster care
  • Crisis intervention
  • Recreational programs
  • Client/family needs assessment
  • Individual treatment plans
  • Case management
  • Medical services
  • Inter-agency service plans
  • Intra-agency referral
  • Educational placement

CLIENTS:

  • Ages 8 through 16
  • Moderately to severely emotionally troubled
  • Able to be safely maintained in a community setting
  • Thirteen client capacity

LENGTH OF STAY:

  • Maximum length of stay 90 days

LICENSE:

  • Department of Human Resources
  • Accredited by the Joint Commission on the Accreditation of Healthcare Organizations (JCAHO)

REFERRALS:
Referrals to Bridges should be initiated through one of the following agencies:

  • Department of Social Services, statewide
  • Department of Juvenile Services
  • Core Service Agencies of Maryland
  • Baltimore Mental Health Systems

All Bridges referrals must be made through the Woodbourne Intake & Referral Department.
