Your employment is contingent upon you supplying the Human Resources (HR) Office with a confirmation receipt which indicates completion of your fingerprint cards performed at the Criminal Justice Information System (CJIS). Before you report to CJIS to be fingerprinted, you must obtain a Preliminary Job Assignment and Criminal History Request/Concentra Medical Exam Request form from the HR Office. The CJIS office is open from 8:00 am - 7:00 pm Monday through Friday, and every 1st and 3rd Saturday of the month from 8:00 am - 4:00 pm. You do not need to make an appointment and there is no charge for this service as long as you display the above mentioned Request form.
